STEP 1:

To start the process, click this link > Online Enrollment

STEP 2:

Print the confirmation page after you complete the online data entry.

STEP 3:

Bring the following documents to the SMHS Registrar to complete the registration process.

- Confirmation page from online enrollment

- Copies of two recent utility bills with parent name and address

o If you do not have utility bills in your name- a Landlord/Homeowner Verification Form will need to be completed and signed by the homeowner. The homeowner will need to provide a recent utility bill. ( Click here to download this form)

- Copy of immunization records

- Copy of transcript/withdrawl grades (does not apply to incoming 9th grade students)

IMPORTANT: REGISTRATION IS NOT COMPLETE UNTIL THE REQUIRED REGISTRATION DOCUMENTS ARE BROUGHT TO THE ENROLLMENT OFFICE.
